Genital PRP (Platelet-Rich Plasma) in Turkey

Genital PRP (Platelet-Rich Plasma) applications are frequently performed, especially in recent years, due to the aesthetic and functional benefits it provides.

Among the purposes of genital PRP injections, there is often relief of menopausal complaints, treatment of lichen sclerosis disease, increasing sexual pleasure, and facilitating orgasm function.

The genital PRP procedure for increasing sexual pleasure and orgasm function is known as “O-shot”.

The PRP procedure is the injection of the "platelet-rich plasma" part in the fair yellow-colored liquid, which is separated from the blood taken from the patient's arm after a special centrifugation process. “Genital PRP” procedure is the application of PRP injection to specific areas in the genital area.

Platelets and Growth Factors

Platelets, also known as "thrombocytes" or "blood flakes", are highly effective in wound healing through the growth factors (GF) they secrete, apart from the blood's coagulation function. With the release of growth factors to the environment;

  • Collagen production increases,
  • New vessel formation is stimulated,
  • Healing occurs with a rapid regeneration process.

Indeed, platelets are the primary source of growth factors that play a fundamental role in wound healing.

The idea of ​​using platelet-rich plasma for therapeutic purposes is also due to the high amount of growth factors in the content of platelets.

Historical Background

The regenerative effect of platelets on tissue regeneration was first demonstrated by Ross et al. in 1970.

The first PRP application was performed by Cardiac Surgeon Dr. Ferrari et al. in 1987 to stop bleeding during an open-heart surgery. Later, PRP treatments took their place in cosmetic and regenerative medicine.

Use of PRP in aesthetic medicine

PRP is also frequently applied in different fields of aesthetic medicine and cosmetic dermatology. Some of these are:

  • Skin rejuvenation (Face, neck, décolleté, hand)
  • Treatment of fine lines and wrinkles
  • Giving fullness
  • Acne (pimple) scars
  • After alopecia (baldness) and hair transplantation
  • Cellulite
  • Striae (stretch marks)
  • Wound healing

What kind of function do the growth factors in the platelet have?

PRP also goes by different names in the literature. Some of those:

  • Platelet-rich plasma
  • Platelet-rich concentrate
  • Autologous platelet gel
  • Platelet releasate
  • Plasma-rich growth factors (PRGFs)

Within the platelets, there are six important growth factors with different functions.

On the other hand, small amounts of leukocytes and antibodies in PRP also enable PRP to act as a physiological antibiotic. Plasma also contains hormones, biotransformed vitamins, and other food sources.

How is genital PRP prepared?

For the procedure, approximately 15-20 cc of blood is taken from the arm first. The volume taken can be increased up to 50-60 cc depending on the desired amount of PRP. This blood is then quickly transferred to special PRP tubes and centrifuged. Three separate layers are formed in the tube after centrifugation:

  • The red part with red blood cells at the bottom
  • PRP part in the middle with platelets and white blood cells (slightly turbid plasma part called 'Buffy Coat')
  • Clear PPP (Platelet Poor Plasma) part at the top.

PRP is obtained from approximately 10% of the blood taken. Thus, approximately 1.5 cc of PRP is obtained from 15 cc of blood. However, if a higher platelet concentration is desired, second centrifugation can also be performed. Thus, the first spin separates red blood cells from plasma, and the second separates PRP from platelet-poor plasma (PPP).

What is O-Shot?

O-Shot is a non-surgical method for sexual enhancement by injecting PRP solution into the genital areas. It’s also known as “Orgasm Shot”. In this procedure, the most sensitive parts in genital region such as clitoral hood area, vulvar vestibular area, anterior vagina are used for the injections. It takes only a few minutes, and the effect starts after 7-10 days. It’s very safe, and the procedure can be repeated on demand of the patient. In our clinic in Istanbul, after preparing the PRP, we activate it by ultraviolet light to get better results.

Genital PRP for the treatment of menopausal problems

The first-line treatment of skin thinning in the genital area and related complaints starting with menopause is locally used estrogen drugs. Estrogen drugs can be given as cream, ovule, or vaginal suppository forms.

However, since it is recommended to stay away from estrogen-containing drugs in people who have been treated for breast cancer or who are at risk of cancer, PRP treatment can be applied as a good alternative method.

We recommend repeating the PRP application 3-5 times at 3-4 week intervals to eliminate the complaints related to menopause. As a result of this treatment, a significant decrease is observed in complaints such as vaginal dryness, burning, tenderness, and pain during sexual intercourse.

Genital PRP for the treatment of lichen sclerosus

Lichen sclerosus is a chronic and progressive skin disease that is thought to be of autoimmune origin and is characterized by thinning of the skin of the genital area. It causes many complaints such as itching, burning, irritation while wearing trousers, pain during sexual intercourse, which impair the quality of life. Especially with the transition to the menopausal period, complaints increase. With PRP applied to this area, a significant reduction in genital lichen sclerosus complaints can be achieved.

Changes in the genital area due to lichen sclerosus:

  • The top of the clitoris has adhered,
  • The head of the clitoris is buried,
  • The inner genital lips are red and erased,
  • The skin texture is quite thin.

According to our observations, combination of fractional carbon dioxide laser and PRP injection is quite effective in the symptomatic treatment of vulvar (genital) lichen sclerosus disease. Treatment can be applied as 2-4 sessions every 3-4 months. Stem cell treatments can also be performed with SVF in advanced lichen patients.

Other application areas of genital PRP

Although it has been stated that genital PRP can be applied in urinary incontinence treatment, interstitial cystitis, bowel incontinence treatment, and chronic pain after mesh surgery, scientific data on these issues are yet to be confirmed. 

In some studies in the literature, genital PRP applications are recommended to increase endometrial thickness and improve ovarian reserve in infertility treatments.
